Tideway: Training Incident Investigation
For Tideway Offshore Contractors TOP developed the training Incident Investigation & Leadership'. A unique combination in gathering knowledge and skills with regard to the execution of relevant and usefull incident investigation, combined with leadership skills of ship's staff and project staff.
The purpose of this training is not to learn how to lead a full and heavy investigation. The training foresees in simple but clear methods and skill, so that the ship's staff and project staff can execute investigation on minor incidents and/ or start investigations in major accidents before the (external) investigation team takes over.
The training contains a theoretic and a practical part with cases. In the endcase, the group is devided in small investigation teams. Each team has to find out what happened, based on limited given information. The teams are challenged to get to know what happend by finding missing links and building up a fault tree and a bow-tie.
